
Our Mission
In 2013, onehome set out to solve a breakdown in a healthcare system that prevents patients from receiving fully coordinated care at home. More than ever, patients want to heal at home, and with 10,000 baby boomers turning 65 every day, demand is only growing. At the time of hospital discharge, too many people are sent to skilled nursing facilities for non-medical reasons, or sent home with inadequate care, only to quickly return to the ER or hospital. This failure stems from misaligned payment models, poorly coordinated home-based services and a lack of focus on quality –- in short, from a system that doesn’t work for today’s – and tomorrow’s – patients. It affects our parents, grandparents, friends and, one day, us. It’s what has led onehome to change the face of post-acute and home-based care.

onehome and its health plan partners recognized early on that a new model designed to get patients on the road to healing at home had to be built. Care in the home needed to be reimagined to meet expanding patient needs, tied to total cost-of-care goals. We developed a vertically-integrated care and benefits-management model and technology platform that solves for the new challenges, validated by our value-based care delivery to nearly 1 million patients. We have met the future with a team, culture and solution that brings healing home.

and benefits management operations, and technology and medical economics. We are the faces of total care in the home.


Ramon A. Falero co-founded onehome in 2013. Under his leadership, onehome created an integrated post-acute homecare distribution and care delivery model that has solved many of the issues prevalent with traditional homecare, aligning managed care organizations, primary and specialty care doctors, and in-patient facilities to provide quality, value-based care for member patients.
With Ramon’s help, onehome has brought infrastructure, systems, and providers under one platform, solving for the coordination, distribution, and provision of care and enabling all aspects of care to be delivered in the home. With Ramon at its helm, onehome strives for improved health outcomes, driving lower costs, enhancing quality, and providing a positive patient experience.
Prior to becoming CEO at onehome, Ramon served as its President. Key initiatives involved acquiring and vertically integrating the company’s home health providers. Through his diverse roles in the healthcare sector, including asset-based financing, middle market lending, and surgical supply and eyecare product distribution, Ramon brings a broad range of experience to onehome.


Cheri Rodgers was invited to co-founded onehome in late 2013. Focused on growth, operations, business development, and new market expansion and health plan relations, Cheri has since helped onehome become one of the world’s largest integrated home benefit management companies.
As onehome’s Chief Operating Officer, Cheri has played an instrumental role in designing new state-of-art software, implementing new payor contracts, overseeing the opening and licensing of various facilities, and building innovative new post-acute care programs. Cheri also continues to develop, acquire, and maintain payor relationships while developing strategic products and innovative programs.
In healthcare since 1989, Cheri has gained a wealth of experience in health plan and managed care operations, primary care physician practice management, integrated home care, and post-acute care.
Cheri began her career with a large managed healthcare plan, taking on a variety of roles during her 10-year tenure before going on to work as the Director of Operations at a publicly traded healthcare company. It was during this time that she was recruited as COO for the newly formed integrated healthcare company, Atenda Healthcare Solutions.
At Atenda, Cheri primarily oversaw subsidiary operations for Home Benefit Management, Specialty Infusion Pharmacy, and Nursing Management Company, while also working closely with the Durable Medical Equipment Company. During her more than 10 years with Atenda, Cheri helped to grow their health plan payor contracts from one to seven contracts covering more than 1.4 million managed care lives. When Univita Healthcare Solutions acquired Atenda in 2009, Cheri remained with the company, continuing to maintain contractual relationships with many of the existing health plans and supporting Univita’s growth.
It was during this time that she was recruited as COO for an integrated healthcare company, Atenda Healthcare Solutions.


Dr. Joseph Mayer joined onehome in 2018, and has headed up such key areas as growth, clinical management (medical management, home health, etc.), and products and technology. Working closely with the board of directors and senior management, Dr. Mayer’s goal is to drive the strategy and trajectory of the business.
After building the growth team from the ground up, Dr. Mayer tackled the restructuring of the clinical division, always striving to achieve quality and excellence in the areas of medical management and innovative clinical programs such as SNF@home.
Dr. Mayer has spent the last 15 years as a clinician (psychiatrist), healthcare executive, and entrepreneur, accruing unique experience as both provider and payer with a focus on technology. Dr. Mayer has worked extensively advising and operating companies working in post-acute care, home-based services, and Medicare Advantage.
As founder and CEO of Cureatr, Dr. Mayer was responsible for driving the company’s mission, vision, and growth, building the enterprise into a leading venture-backed healthcare technology company that delivers value-based solutions around real-time data exchange to support care transitions, medical management, and provider communication.
Prior to founding Cureatr, Dr. Mayer trained and worked as a psychiatrist at the Mount Sinai Medical Center in New York City and operated a healthcare informatics consulting firm, Inomaly, which focused on healthcare data loss prevention and clinical decision support.
With an M.D. from Columbia University and a Crain’s 40 Under 40 Award under his belt, Dr. Mayer is on the board at Cureatr and is a managing partner at Diagram Healthcare.


Ryan Cochran joined onehome as Chief Financial Officer in 2018 and helped recapitalize the business to focus on continuing to provide a high level of customer service while expanding its markets and offerings. Since then, Ryan has played a key role in developing company strategy; growing onehome’s offering with new payer partners, medical economics, and analytics; optimizing operations, including revenue cycle, procurement, and reporting; and reporting on and collaborating with various constituents.
Ryan has made a career of working as an operator, investor, and advisor to innovative healthcare provider, payer, and technology companies. Prior to joining onehome, Ryan held leadership roles at private-equity backed health insurance plans and integrated delivery systems including Bright Health, Clover Health, and CarePoint Health.
As a healthcare investor at New Leaf Venture Partners, a leading growth-oriented healthcare investment fund, Ryan helped initiate investment in the tech-enabled healthcare services sectors. Several of Ryan’s investments led to acquisitions by major payer and provider organizations, as well as initial public offerings.
Prior to New Leaf Venture Partners, Ryan worked in healthcare investment banking at Citi, where he advised companies on mergers and acquisitions as well as other strategic transactions.
Ryan holds a B.S. in economics and a B.A. in biology from the Wharton School of the University of Pennsylvania, as well as a graduate certificate degree in Healthcare IT from Columbia University College of Physicians and Surgeons.


Laurie Colasanti joined onehome as Chief Human Resources Officer in 2020, where she is responsible for all aspects of the organization’s human resources. Over the course of her 25 years in human resources, Laurie has worked in the automotive, healthcare, transportation, and petrochemical industries.
Leveraging her extensive domestic and global experience in human resources management and organizational development, Laurie has led various HR operations, handling restructuring, divestment, workforce scaling, merger and acquisition integration, culture change, and people development for small and large organizations alike.
Laurie’s areas of interest and expertise include all capacities of HR operations, strategic planning, organizational design, development and structure, labor negotiations, front line communications, inclusion and diversity, company growth and expansion, and strategic change management.
Laurie has held executive HR positions in several large-scale organizations. Her achievements include heading up HR globally for various businesses within LyondellBasell, a $50B petrochemical multinational; directing HR for GKN, a $12B Tier 1 global automotive supplier; and acting as Vice President of HR for DHL Express Americas. Prior to joining onehome, Laurie led human resources for Sixt rent a car’s expansion into the United States.
Laurie earned her B.A. in business and psychology at Oakland University before going on to do an M.A. in organizational psychology at Central Michigan University.


Albert Asis joined onehome as Chief Information Officer in late 2013. Responsible for the design and architecture of the company’s proprietary software, onecare, Albert also oversees all of the company’s other technology initiatives. Providing support and guidance for application development processes companywide, Albert uses his expertise to steer the overall technology direction for onehome’s healthcare technology products.
Albert has more than 22 years of technology experience in a number of different companies, the majority of which have been in healthcare. Prior to joining onehome, Albert spent three years working as a product architect at Univita (formerly Atenda Healthcare). Albert has also held a variety of other positions, including Senior Architect, Chief Technology Officer, and VP of Software Development.
Albert has a master’s degree in computer science and mathematics from Bar Ilan University in Israel. Albert currently resides in Miami with his wife and two wonderful kids.


Dr. Niraj Patel joined onehome in 2019 and is currently Senior Vice President of Medical Affairs, working closely with all units of the organization. In his previous role as onehome’s Chair of Care Management, Dr. Patel was responsible for managing strategic planning and growth (UM, CM, DM), implementing best practices for effective coordination of UM, CM and care (divisions), and overseeing regulatory compliance and quality assurance. His natural leadership abilities make him perfectly suited to taking on team building and senior leadership development initiatives, not to mention staff training and supervision.
Dr. Patel has extensive experience heading up risk-bearing entities for commercial-, state-, and federal-sponsored health, particularly as it pertains to population health analytics and management. Before joining onehome, Dr. Patel acted as Senior Medical Director at myNEXUS, a home health benefits management organization, where he helped manage contracts for many Fortune 500 health plan clients. Dr. Patel also served as Chief Medical Director for Optum-UMR, where he partnered with senior-level executives, clinical managers, , and operational staff to optimize clinical and business solutions through reporting and program and benefit management, all in compliance with NCQA and URAC standards.
Dr. Patel holds an M.D. from Ross University School of Medicine as well as a Master of Health Administration (with a focus on population health and analytics) from the University of North Carolina at Chapel Hill.


Jorge Bustillo joined onehome in 2015 as Vice President of Operations. Equipped with 22 years of experience in healthcare operations, Jorge has a demonstrated history working in the healthcare industry. His expertise in operations, project management, , and product development have made him an invaluable asset to the organization.
While with onehome, Jorge has helped develop key organizational processes and protocols that allow for freer and more effective collaboration between departments. Jorge also created the Project Management Office, which oversees all organizational projects and ensures their implementation
Jorge holds a bachelor’s degree in history from Florida Atlantic University as well as an MBA with a specialization in healthcare management from St. Thomas University.


Nicolas Lievano, a strategic economist, joined onehome in 2019. As Vice President of Medical Economics, Nicolas aims to apply his extensive experience in analytics and data visualization to helping onehome become a data-driven care organization able to optimize the use of econometrics and advanced statistical approaches.
Prior to joining onehome, Nicolas held executive positions at Community Medical Group and at New Century Health, where he helped address a wide range of business objectives including product development, new market strategy, cost-effectiveness, and quality of care.
A seasoned analyst and business executive with a proven record leading high performing teams, Nicolas is a welcome addition to the onehome team. His wealth of experience in managed care finance, P&L analytics, predictive modeling, contracting, business development, financial analysis, forecasting and budgeting, strategic and clinical initiatives, physician compensation, and population health management further attest to the value he brings to the organization.
Nicolas holds an MBA in health sector management and policy from the University of Miami, as well as a BASc in economics from Arizona State University.


Jennifer Southwell joined onehome in 2017 as Director of Home Health Services before going on to become the division’s Vice President two years later. During her time at onehome, Jennifer has helped build and oversee four Home Health agencies and has been a key contributor to clinical programs such as . Jennifer’s decisive roles in clinical team and program oversight, delegation compliance, and business development make her an invaluable asset to all of onehome’s Home Health Services.
Jennifer’s extensive history working in the health care sector is accentuated by her strong focus on rehabilitation in the home and in outpatient settings. As a licensed and registered occupational therapist, Jennifer has perfected her skills in home healthcare setup and administration and put her knowledge of compliance, best practices, billing, utilization management, clinical programs, and day-to-day operations to excellent use. And she has always been a warm, comforting presence in the lives of her patients.
Jennifer holds a degree in occupational therapy from Florida International University. Prior to joining onehome, Jennifer was owner and operator of On The Way Home Care Services, an agency she maintained for 10 years.


Nelson Vegas joined onehome as the Vice President of Durable Medical Equipment in 2015, providing leadership and guidance to ensure that all medical equipment, supplies, and services are delivered to a patient’s home accurately and on time. Working to improve patient services in collaboration with onehome leadership, health systems, and referral sources, Nelson has established and implemented new policies and procedures for departmental operations, helped develop brand new fulfillment centers in expansion markets, and played a key role in the organization earning Joint Commission accreditation.
A powerful force in the workplace, Nelson uses his positive attitude and tireless energy to encourage others to work hard and succeed, ensuring that all departments meet quality standards, inventory levels, safety guidelines, CMS compliance, and customer service expectations.
With more than 20 years in the DME field, Nelson has a wealth of experience spanning DME operations, purchasing, procurement, revenue cycle management, and project/program management.


Arnie Finkel joined onehome in 2014. Arnie was among onehome’s first employees. Tasked with building the IV pharmacy and team from the ground up and presenting a fledgling organization to insurance companies via RFPs, Arnie played a pivotal role in onehome winning several contracts. Under Arnie’s skillful management, the onehome pharmacy is now triple accredited.
Prior to joining onehome, Arnie spent eight years on the executive team at Atenda, where he was responsible for daily pharmacy operations and related patient care services, as well as overseeing compliance with all state, federal, and JCAHO regulations.
A pharmacist for 30+ years, Arnie has spent more than half of his career specifically focused on the home infusion industry, in which he has held management and administrative positions at hospitals and home health agencies. Arnie also owned and operated four retail pharmacies for 16 years, during which time he was able to hone the skills required to run and manage a business.


Veronica Cope-Visco joined the onehome team in 2020. She is responsible for directing a team of licensed professionals (RNs, LPNs, social workers, OT/PT specialists) and overseeing such instrumental functions as care coordination, case management, and utilization management of home health services, home infusion, and DME. Veronica uses reliable methods to measure and evaluate program quality and costs and ensure that they are utilized in the most effective way possible.
Veronica has been instrumental in implementing utilization management practices that adhere to evidence-based guidelines as well as federal, state, and health plan medical necessity criteria. Her knowledge of population health management, case management services, and medication management have been a true asset in the development of policies, procedures, and standards that drive effective management of care.
Veronica is a medical management professional with a wealth of knowledge and leadership experience in medical, care, and population health management; her quality of care and patient satisfaction results speak for themselves.
Prior to joining onehome, Veronica served as Senior Director of Medical Management, Quality, and Reimbursement for the Health Care District of Palm Beach County’s Medicaid, Healthy Kids, Health Flex and Indigent health plan programs. Most recently, she was Director of Care Management at Pediatric Associates, where she established a quality and population health and care management program and helped the organization to obtain medical home and network accreditation.
A registered nurse, Veronica received her B.Sc.N. from the State University of New York College at Plattsburgh.


Dr. Michael Syme joined onehome as Medical Director in 2019, providing services including authorization review, clinical and cost data analysis, and quality-focused program development to reduce acute care utilization. Dr. Syme also created onehome’s Wound Care Bundle Program to improve home wound care and clinical outcomes.
In 2020, Dr. Syme took on the daunting task of the organization’s COVID-19 Pandemic Response Protocol and Guideline Development. Leading the company-wide COVID-19 Clinical Response Team, Dr. Syme succeeded in developing and implementing policies and procedures to safeguard both employees and patients.
In addition to serving as onehome’s Medical Director, Dr. Syme is a telemedicine physician with Teladoc Health, providing virtual primary care and urgent care consultations for 300+ patients per month, as well as an after-hours physician with VITAS Healthcare, providing end-of-life care and palliative care. Prior to joining onehome, Dr. Syme provided adult in-home primary care and urgent care services for EliteHealth and was a strategy and operations consultant at Humana.
A graduate of the Florida International University Herbert Wertheim College of Medicine’s inaugural class, Dr. Syme also holds a B.Sc. in psychology from the University of Florida. Dr. Syme is board certified with the American Board of Internal Medicine and completed the Internal Medicine Residency Program at Mount Sinai Medical Center in Miami Beach.
