In 2013, OneHome set out to solve a breakdown in a healthcare system that prevents patients from receiving fully coordinated care at home. More than ever, patients want to heal at home, and with 10,000 baby boomers turning 65 every day, demand is only growing. At the time of hospital discharge, too many people are sent to skilled nursing facilities for non-medical reasons, or sent home with inadequate care, only to quickly return to the ER or hospital. This failure stems from misaligned payment models, poorly coordinated home-based services and a lack of focus on quality –- in short, from a system that doesn’t work for today’s – and tomorrow’s – patients. It affects our parents, grandparents, friends and, one day, us. It’s what has led OneHome to change the face of post-acute and home-based care.
OneHome and its health plan partners recognized early on that a new model designed to get patients on the road to healing at home had to be built. Care in the home needed to be reimagined to meet expanding patient needs, tied to total cost-of-care goals. We developed a vertically-integrated care and benefits-management model and technology platform that solves for the new challenges, validated by our value-based care delivery to nearly 2 million patients. We have met the future with a team, culture and solution that brings healing home.
