Here is an overview of what to expect.
You’ll receive a referral from your primary care provider, hospitalist or other doctor for home healthcare, home medical equipment, home infusion or all of these services.
OneHome will call your doctor and you to confirm, discuss your needs and schedule an in-home medical visit with a OneHome team member.
During your first home healthcare, or home infusion visit, you will talk to a nurse who will ask about your health challenges, needs and goals for recovery and educate about OneHome’s services, while providing any necessary medical care or medications. We’ll go over important questions, your medical history and medications, and we’ll complete the forms, which are similar to those you sign in your doctor’s office, that allow OneHome to treat you. We’ll then schedule a future visit.
A OneHome teammate will also deliver your home medical equipment, answer any questions you may have about it, and provide you with contact information should you need a repair or have additional questions. Keep in mind that information on your OneHome visits is sent to your primary care doctor and your health plan case manager.
Once you are a OneHome patient, you can reach us any time of the day or night, including weekends and holidays, and we will continue to coordinate and provide all of your home-based services.
