Slippers at hospital vs home

Healing faster and better at home.

People recover faster at home, where they have the emotional support of family and the comfort of familiar things. It’s a more human approach to getting well.

We’re here to serve you.

onehome teammates come to your home immediately after a hospital or nursing home discharge or referral from your doctor to provide what is needed for you to heal at home.

3. First visit

During your first home healthcare, or home infusion visit, you will talk to a nurse who will ask about your health challenges, needs and goals for recovery and educate about onehome’s services, while providing any necessary medical care or medications. We’ll go over important questions, your medical history and medications, and we’ll complete the forms, which are similar to those you sign in your doctor’s office, that allow onehome to treat you. We’ll then schedule a future visit.

4. Home medical equipment

A onehome teammate will also deliver your home medical equipment, answer any questions you may have about it, and provide you with contact information should you need a repair or have additional questions. Keep in mind that information on your onehome visits is sent to your primary care doctor and your health plan case manager.
